Kenneth Young Center reaches out to older adults in Elk Grove and Schaumburg Townships to assist them in identifying resources and availing themselves of supports to maintain their independence in the community for as long as possible.

To accomplish this goal, KYC provides a unique variety of services that offer the information, support and encouragement needed to maximize seniors' autonomy.

Seniors Finding Solutions

A Private Care Management Program for Seniors

from Kenneth Young Center, with a 25-year record of high quality senior care management, helping people feel good, do better and find solutions

Seniors Finding Solutions is a new care management program to help you and your family find solutions to the challenges of growing older at home. We incorporate individual, family and community resources to find the best possible solutions for you and those who care about you.

Most seniors want to remain in their homes and in control of their own decisions as long as possible. Our Kenneth Young Center care management staff is uniquely qualified to coordinate the information, support and encouragement needed to maximize an individual’s independence and the caregiver’s well being.

Our experienced staff will:

« Visit clients and their families at home to assess the needs of the individual

« Develop a tailored care plan to meet those needs, considering the individual’s wishes, family preferences and resources

« Arrange and monitor services to help clients stay independent and connected to their community for as long as possible

Care management includes:

« Linkage to services including homemakers, home companions and social activities

« Monitoring of quality services

« Serving as a liaison between family and other professionals

« Caregiver support

« If you live out of state, Seniors Finding Solutions will assure you that your relative is safe and receiving the best care and assistance with daily life management decisions.

  • If you live nearby, you can be involved in your relative’s care and still attend to the other dimensions of life for you and your family.

Seniors Finding Solutions services are tailored to each individual and conducted in a friendly, professional manner by qualified case managers with years of expertise helping seniors overcome obstacles to recapture the real meaning and vitality of everyday life.

Seniors Finding Solutions is a fee-based service of Kenneth Young Center.
